We think of "health" defined very broadly – not just helping people when they get sick but helping them feel fully engaged in a community where they feel they belong and fully engaged in activities that promote their well being.
Model of Health and Wellbeing
Stonegate Community Health Centre, as one of a network of CHCs and Aboriginal Health Access Centres (AHACs) across the province, is committed to a new Community Health Centre and Aboriginal Health Access Centre Model of Health and Well Being.
CHCs and AHACs offer a range of comprehensive primary health care and health promotion programs in diverse communities across Ontario. Services are structured and designed to eliminate system-wide barriers to accessing health care and harmful forms of social exclusion that may lead to an increased burden or risk of ill health. Although each CHC and AHAC across the province is different, because they respond to the specific needs of the communities they serve, each follows the same model to promote health and wellbeing.
This model is based on two core principles adapted from the World Health Organization (WHO):
1) The enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being.
2) Health is a state of complete physical, mental, social and spiritual well-being and not merely the absence of disease or infirmity.
