Client Support Services
Information is readily available on Government Support Programs, such as Wheeltrans, Trillium Drug Plan). Our Team also assists in filling out government applications, which may include Permanent Resident Card Renewal, Interim Federal Health Program (IFHP) form for refugee claimants, and Ontario Works Program.
Stonegate CHC’s Housing Support and Case Management Program is geared to residents in the community facing difficulties in accessing housing and/or maintaining their current accommodation.
The Program advocates for residents with barriers in accessing community resources, such as financial assistance and emergency support to improve their social health.
Our Team assists in connecting tenants with landlords, outreaching in the community to help find safe and stable housing for individuals in need. Services include completing applications for Toronto Social Housing, providing listings of rooms or apartments in price range, referrals to Homes for the aged, CCAC (Community Care Access Centre) and support housing.
Individuals can access information on tenants’ rights and responsibilities, and receive advocacy on tenant related issues. Specifically, our Team assists in mediation to resolve client conflicts with landlords, follow-up on satisfactory level of maintenance and repairs, and help individuals access income support (e.g. Ontario Works, Ontario Disability Support Program, Employment Insurance, Old Age Supplement).
Eviction Prevention & Emergency Support
Our Team is available to help individuals facing eviction, discrimination and harassment. Specifically, priority is given to women and individuals requiring immediate housing due to abuse or domestic violence. Individuals can seek guidance on how to apply to the Ontario Rental Tribunal, and be referred to external agencies such as emergency shelters and food banks, Community Legal Services, Social Services or Rent Bank, among others.

Settlement Services for Newcomers
Experienced settlement workers and other staff can provide information and orientation, referral, solution focused counselling, advocacy and assistance with filling out forms and accessing government services for newcomers to Canada.

Income Tax Clinic
Stonegate CHC is committed to helping clients and community members address issues related to poverty and income. Our strategy includes offering free help with annual income tax returns and help to access all the income supports and tax credits you may be entitled to.
