West Toronto Diabetes Education Project
Stonegate CHC is a member of the West Toronto Diabetes Education Project (WTDEP), a partnership of 5 Community Health Centres, 3 hospitals, and 2 social service agencies. A health care team consisting of a Diabetes Nurse Educator and a Registered Dietitian provides individual diabetes management, education, and counselling services to Stonegate CHC clients. Referrals to the program are made by primary care providers, and clients can book and access diabetes services onsite.
Individual counselling is available to community members that are not clients of Stonegate CHC. Referrals are welcome from local doctors to Stonegate CHC for more comprehensive diabetes management and counselling services. The WTDEP team ensures that appropriate follow-up and continuity of care is made with all clients.
In addition, diabetes education is also offered in group formats. Group sessions are open to all community members affected by diabetes, and caregivers are welcome to attend and participate in the learning.
