Family Practice Clinic

Stonegate CHC offers a complete Family Medical Practice.

Primary health care services are provided by a team that works together to care for your health.  Our team includes: Family Doctors, Primary Care Nurse Practitioners, a Registered Nurse and a Registered Practical Nurse who provide medical care to all members of the family.  They have a special interest in how life in your community affects your health. Each patient is assigned a primary and secondary Health Care Provider who shares responsibility for your care.  Our services include illness prevention, disease management and care of chronic illness, pre and postnatal care for mothers and their newborns, and complete physical examinations.

Primary healthcare is available to registered clients only (see "Becoming a Client").
Stonegate CHC is not a walk-in medical clinic and does not provide emergency services.

 

Rehabilitation Programs

New Rehab Program for those with Arthritis: GLA:D Good Living with Arthritis Denmark (GLA:D®) an 8 week rehabilitation program for people with Hip or Knee arthritis. This program helps individuals to manage the symptoms of hip or knee osteoarthritis (OA).  The program is being made available free of charge.  The GLA:D program has been shown to help people with hip and knee OA reduce pain and stiffness and become more physical activity.  The GLA:D® program includes:

- 12 exercise sessions:  2 times per week for 6 weeks; GROUP exercise sessions with up to 8 people, to be completed within 8 weeks
- 2 education sessions:  1.5 hours each 

When you have hip or knee OA, the muscles around the joint can become weak and cause changes to the position of the joint. This can be part of the reason you have pain and difficulty moving. The GLAD program teaches you how to control your movements to make your muscles stronger and reduce stress on the joint. Research has shown that 12 sessions over 6 weeks is needed to make lasting changes to your muscles, movement, pain and function.

New Pulmonary Rehab Program for those with COPD:   Living Well with COPD-is a free Pulmonary Rehabilitation Program.  It  includes exercise and education for people with Chronic Obstructive Pulmonary Disease (COPD).  Exercise is supervised and designed to improve balance, strength and cardiovascular health.  A multi-disciplinary team of professionals will facilitate the education sessions.  The Team includes a Certified Respiratory Educator, Physiotherapist, Dietitian, Occupational Therapist, Social Worker and Pharmacist.  Educational sessions will include a wide variety of COPD related topics from medications and vaccination to nutrition and anxiety management.  The program runs for 8 weeks, twice per week.

This program is offered through a partnership with Stonegate CHC and The Four Villages CHC.

Smoking cessation support, including access to free nicotine replacement therapy, is available to everyone in the community at Stonegate CHC as well.  Call 416-231-7070 for more information about this program or take this form to your primary care provider who can make a referral.

 

Smoking Cessation

Specialized training and resources allow Stonegate’s health care providers to offer counselling and support to individual clients and community members contemplating and or attempting to quit smoking. Based on the stages of change principles, health care providers work with clients to address the destructive effects of tobacco use and the associated illness and disease. Access to free nicotine replacement therapy and resources support those clients and community members impacted by low-income. 

 

Asthma Education

Our Certified Respiratory Educator can provide you with helpful information to understand your asthma or other breathing problems so that you can manage your condition better and improve your quality of life.  The Asthma Education program is available to individuals, families, and community members in one-to-one sessions and group education sessions.  A Certified Asthma Educator will help you understand your level of asthma, triggers, and criteria for asthma control.  You can learn how to use asthma medications properly and better understand the role of various medications in asthma management.  We will give you the tools you need to monitor your asthma at home, and provide access to free educational materials.

These services are offered at Stonegate CHC and through partnerships at three additional Community Health Centres – Parkdale CHC, Unison CHC and LAMP CHC.

The 2009 Ontario Health Quality Council Report identified the Primary Care Asthma Program (Stonegate CHC was a founding site) as one of the 10 things that is working best in Ontario Health Care. Research indicated a 50% drop in emergency department visits for children and a 49% drop in school absenteeism among asthmatic children.

 

Chiropody - Foot Care

Our Registered Chiropodist specializes in the diagnosis and comprehensive care of foot and leg problems. We focus on offering care to seniors, people with disabilities, adults and children who have underlying medical conditions such as diabetes, dermatological problems or other foot care needs. Our Registered Chiropodist provides complete foot care and treatment services, including ulcer care, on-going nail care, corn removal, callus reduction, orthotics (charges may apply), and advice on buying and fitting shoes. The Chiropodist also supports good foot health through individual counselling and community education workshops.

 

Nutrition

Our Registered Dietitian offers nutrition education and counseling throughout the whole lifespan, from infant to older adult. Our focus is not only management of disease but also prevention of illness through a nutritious diet and a healthy relationship with food.  Our dietitian works from a non-dieting and a non-judgmental approach with a focus on health and overall well-being rather than weight. The sessions offered are one-on-one or in groups with clients and community members to help them meet their unique needs and address their nutrition and health concerns. Our dietitian also provides variety of workshops and education events related to food and nutrition.

Registered Dietitian provides free individual nutrition counseling for:

  • high cholesterol                     
  • high blood pressure
  • pre-diabetes and diabetes
  • iron deficiency anemia
  • preconception, pregnancy and breastfeeding
  • picky eating in children
  • digestive problems 
  • osteoporosis
  • weight concerns
  • weight preoccupation

  • disordered eating
  • repeated dieting
  • vegetarian & vegan diets
  • other nutrition issues

FoodFit Program: Better health, one step at a time  

Starting in April, 2019, Stonegate CHC's dietitian will be running the FoodFit program. FoodFit is a free cooking program available to people of all ability levels. It is focused on building nutritional literacy and promoting healthy habits, while helping balance one's diet with tasty, affordable recipes. We work on increasing physical activity levels by focusing on daily walking as a form of easy and cost-free exercise.

FoodFit is a 12-week program, meeting once weekly to engage in a short discussion with light snacks, a bit of light physical activity that usually involves a short walk, and most important cooking a shared meal from scratch using fresh ingredients. A typical workshop lasts 3 hours, the bulk of which is spent in the cooking class (and eating!) portion.

Benefits:

  • Learn how to cook a variety of cuisine styles using healthy recipes. Participants also receive a free cookbook.
  • Learn the benefits of regular physical activity, with a focus on walking as a way to stay fit. Participants also receive a free pedometer.
  • Learn about nutrition and health in general through weekly discussions on certain topics.
