Although we have had to make some changes in how we deliver our services, we want the Stonegate Community to know that we are all still actively working at Stonegate CHC. We want to know if you have any concerns or needs that we might be able to help with. Please call!
Physical distancing is a must to stop the spread of Covid-19, but social connection is still vitally important – we just need to find ways to connect over the phone or through video chats. Our Facebook page is updated regularly with resources and some of our groups and physical activity programming brought to you virtually.

Below is a description of the modifications we have made while continuing to deliver our services:

Stonegate CHC Hours:Our hours are modified Monday to Friday 8:30 a.m.to 4:00 p.m. to ensure adequate staffing complement. 

Please note:  Appointments must be scheduled with a provider, if you need to come to the Health Centre your provider will notify you.

Client Screening: All clients will be screened first on the phone and then by staff behind the glass barrier at reception for symptoms. They will be instructed to sanitize their hands and don a mask. Waiting room chairs have been minimized and spaced to ensure 2 metre distance. Waiting room will regularly be cleaned by staff wearing full PPE.

Staff Screening: All staff are screened for symptoms and have their temperature taken at the beginning of each day.

Primary Health Care: We have suspended non-essential visits to the Centre. Clients who call for an appointment with a primary care provider will be given an appointment for the provider to telephone or have an OTN virtual visit with the client. If the provider decides the client needs to come in for blood work or physical assessment, the provider will schedule the client to come in. Well Baby and Well Child visits are complying the advice from the College of Family Physicians. Prescription renewals continue normally.

Sexual Health Clinic: Clients who call for an appointment will be screened by a nurse over the telephone. If they determine the client needs to come in for blood work or physical assessment, a visit will be scheduled for the client to come in.

Psychiatric Consultation: Psychiatrists are using telephone visits and will have OTN set up in coming weeks. If the psychiatrist chooses to come in they will use a larger meeting space with a table in between client and doctor to maintain appropriate distance.

Chiropody: All Non-Essential visits have been suspended. Emergency visits will continue, i.e. wound care, diabetic foot care.

Dietitian: Telephone or Virtual visits are available. Private FoodFit Facebook group created to allow group to meet virtually and live-streaming.

Physiotherapy: Non-essential visits suspended. Urgent needs being addressed. Some telephone support and Virtual Rehab is continuing.

Asthma: No spirometry – telephone encounters continuing. Clients can pick up nicotine replacement and get counselling over the phone. COPD Rehab suspended, clients can be referred to West Park’s extensive video library. Arthritis Society: Offering Virtual Care for individualized consultation and group education.Clients can call 1-800-321-1433 ext. 3381 to book an appointment.

Diabetes Education: Telephone or virtual visits continuing. Group activities are suspended.

Mental Health Counselling: Clients who call for counselling will be offered a telephone or OTN Virtual appointment. A date, time and phone number to be booked with the counsellor. If counsellor decides an in person visit is required they will maintain 2 metre distance from the client.

ECC-Walk in counselling: Etobicoke Children’s Centre is offering “virtual walk-in” for clients 0-24, their families and caregivers. Clients call 647-272-8482 to book a session. Hours are: Monday-Friday 8:30-4:30

Client Support: Clients who need support with housing, forms, applying for EI or other benefits will be provided with over the phone support. People calling for intake will have a phone consultation about their immediate needs.

Over the phone interpretation: The interpreter can be linked into phone calls with clients.

Nutrition and Food Access Programs: All group nutrition programs have been suspended. Good Food Market suspended.
We are offering to our most vulnerable clients, food vouchers for those who can shop and Good Food Boxes that will be delivered to people’s homes (for those who are self-isolating). For more information, please call Julia at 416.231.7070 ext. 229 or Sonia at ext. 239. 

EarlyON Programs: All in-person EarlyON programs for children 0 to 6 years of age have been suspended. Staff are live streaming programming through our Facebook page – Song Circles, Infant Massage, etc. Staff will be calling participants to explore their needs and to share ideas for activities with children and resources that may be helpful for young families.

Seniors Programs: Group programming is suspended. Staff will be calling participants to explore their needs and to share resources and health information.

ESS –Shopping Trip: ESS suspended this trip. ESS Support Services will be delivering our emergency food to our clients who need that support.

Health Promotion Programs: All group programs have been suspended. Staff will be calling participants to explore their needs and to share resources and health information. Staff are live streaming programming through our Facebook page – Mindfulness, Song Circles, Infant Massage, etc.

Tax Clinic: Essential Services only. We will try to facilitate phone consultation for this in weeks ahead.

Physical Activity Programs: Group programming suspended. Staff will be calling participants to explore their needs and to share resources and health information. Physical activity resources have been posted on our website, newsletter and facebook. Live streaming of programming including qi gong and yoga is being live streamed by group leaders with links on our Facebook page.

TPH-Dental Care: Toronto Public Health suspended all non-emergency visits. TPH staff are re-deployed at other sites and are providing emergency dental care

Early Abilities-George Hull Speech and Language: George Hull has suspended this programming at Stonegate CHC.

Settlement Services: LAMP CHC has suspended this programming at Stonegate CHC
