Important Information Regarding Programs & OHIP Cards


An OHIP card reader has been installed at program reception. We ask that each time you attend an appointment or registered program that you present your OHIP card.

Each person that attends a registered program will have a chart on our database. When you attend program or a scheduled appointment and your OHIP card is swiped on the card reader, it will be documented in your chart that you are participating in Stonegate CHC Health Promotion Programs. The process is similar to when you see your physician.

Flu Vaccine at Stonegate CHC

We have a stronger vaccine for seniors compared to what you can get at a pharmacy.  So come here for your vaccine!

We recommend the flu shot for everyone 6 months old and older.  Kids get the flu most often and kids under 5 are at the greatest risk of complications.

Stonegate has been awarded a grant from the Canadian Tire Jumpstart Charities to support our Martial Arts Rocks programming.

When we first began to offer the Martial Arts Rock program in October 2015 we could not have imagined the interest and enthusiasm of children attending.  Our Martial Arts Rocks programs keeps growing and growing!  We started the program with 10 children attending on a weekly basis and now have 36 registered. New Pulmonary Rehab Program for those with COPD

Living Well with COPD-is a free Pulmonary Rehabilitation Program.  It  includes exercise and education for people with Chronic Obstructive Pulmonary Disease (COPD).  Exercise is supervised and designed to improve balance, strength and cardiovascular health.  A multi-disciplinary team of professionals will facilitate the education sessions.  The Team includes a Certified Respiratory Educator, Physiotherapist, Dietitian, Occupational Therapist, Social Worker and Pharmacist.  Educational sessions will include a wide variety of COPD related topics from medications and vaccination to nutrition and…

New Rehab Program for those with Arthritis

GLA:D Good Living with Arthritis Denmark (GLA:D®) an 8 week rehabilitation program for people with Hip or Knee arthritis. This program helps individuals to manage the symptoms of hip or knee osteoarthritis (OA).  The program is being made available free of charge.  The GLA:D program has been shown to help people with hip and knee OA reduce pain and stiffness and become more physical activity.  The GLA:D® program includes:
