Asthma Education
Our Certified Respiratory Educator can provide you with helpful information to understand your asthma or other breathing problems so that you can manage your condition better and improve your quality of life. The Asthma Education program is available to individuals, families, and community members in one-to-one sessions and group education sessions. A Certified Asthma Educator will help you understand your level of asthma, triggers, and criteria for asthma control. You can learn how to use asthma medications properly and better understand the role of various medications in asthma management. We will give you the tools you need to monitor your asthma at home, and provide access to free educational materials.
These services are offered at Stonegate CHC and through partnerships at three additional Community Health Centres – Parkdale CHC, Unison CHC and LAMP CHC.
The 2009 Ontario Health Quality Council Report identified the Primary Care Asthma Program (Stonegate CHC was a founding site) as one of the 10 things that is working best in Ontario Health Care. Research indicated a 50% drop in emergency department visits for children and a 49% drop in school absenteeism among asthmatic children.
Smoking Cessation
Specialized training and resources allow Stonegate’s health care providers to offer counselling and support to individual clients and community members contemplating and or attempting to quit smoking. Based on the stages of change principles, health care providers work with clients to address the destructive effects of tobacco use and the associated illness and disease. Access to free nicotine replacement therapy and resources support those clients and community members impacted by low-income.
