Please note Online Booking is for current primary care clients that have OHIP.
• Currently you can only book an appointment with Dr. Shannon Cohane, Teresa Cristina Hermenegildo and Crystal Lawrence.
• If this is an emergency, please go to your local ER.
• If you need to book an appointment for a Flu shot, General Check-up/Physical, Well Baby/Child visit or if it is urgent, please call our office @ 416-231-7070 and a Medical Office Assistant will book your appointment appropriately.
• If you are booking an appointment for a family member, please have them book their own appointment if they are over the age of 16yrs old
• Each person will need their own appointment slot
• If the online booked appointment does not meet the criteria, the appointment will be cancelled. We will make our best efforts to contact you but in the event that we cannot, we will re-direct on arrival as appropriate.
• If you need to change or cancel your appointment, please call our office at least 24hrs prior at 416-231-7070 for assistance
