Why we collect information from you.
Stonegate CHC collects information about you so that we can: provide care or services to you; evaluate, manage and plan our services; inform you about upcoming events and news; provide information to funders for health system management purposes; participate in research; and meet our legal requirements.
We ask for your agreement to collect, use and share your personal information.
We will not disclose your personal health information for any purposes other than those listed above, or without asking for your permission except as legally required.
We have a duty to report.
We are required by law to disclose information, with or without your consent, about certain infectious diseases, child abuse and neglect, and instances where there is the potential of harm to yourself or others.
We have responsibility to inform you in case of a breach of.
Stonegate CHC is responsible for the information it holds about you and has policies about the privacy of this information. If there is ever was a breach of privacy we would immediately report this to you.

A staff person at the Centre will provide you with information about our privacy policies and practices if you ask.
You own your personal health information, what are your rights?
You can access personal health information we have about you by making a request in writing. You can also request to change the information we have about you, and restrict who at Stonegate can see that information. We will respond to any request within a reasonable amount of time (no more than 30 days).
Our Privacy Officer can respond to your concerns and questions.
