At this time Stonegate is opening its Primary Health Care Waitlist to eligible family members of current primary health care clients who live in the same household.
To be eligible to become a patient of our Family Practice, you must:
- Not currently have a primary care provider;
- Live within the area bounded by the Humber River to the east, the Queen Elizabeth Way to the south, Kipling Avenue to the west and Eglinton Avenue to the North;
- And, if a person is non-insured they must not have status (e.g. not be on a Visitor’s’ VISA, Student VISA, a VISA holder, on a work permit, on vacation, or a tourist).
