RETIREMENT ANNOUNCEMENT – BEV LEAVER

February 1, 2021

After an exceptional career in community service and thirteen years as Executive Director of Stonegate CHC, Bev Leaver has announced her decision to retire this year.

Under Bev’s leadership, Stonegate CHC has experienced considerable growth and navigated significant changes and advancements. In addition to providing excellent leadership in health care and service to the Stonegate community, Bev was instrumental in securing capital support and overseeing the design and construction of a beautiful new Health Centre. The new Centre has allowed Stonegate CHC to greatly expand its partnerships and deliver a wider range of health care and services to the Stonegate community.

The Board of Directors of Stonegate Community Health Centre is engaged in the recruitment process to find the next Executive Director. In order to ensure a smooth transition for Stonegate CHC, Bev will continue in her role as Executive Director until a suitable candidate is found.

Stonegate CHC New Enhanced COVID 19 Measures

January 19, 2021

Due to rapidly increasing number of Covid-19 cases in the City, effective Thursday January 21st, 2021 we are implementing increased measures at Stonegate CHC to ensure the health and well-being of our clients and staff .

We will be open to in-person appointments only on Monday, Wednesday and Friday. Telephones will be answered every weekday and Telephone or Virtual visits will be offered every weekday.

All appointments will be booked over the phone. Do not come to the centre to schedule an appointment with a staff member, please call.

We are continuing to book essential appointments only. The appointments will start with a phone call or video visit. If the provider decides you need to be seen in person, you will be given an appointment time to be seen on-site.

All clients with booked for onsite appointments will have to complete a Covid-19 screening before coming to the centre.

All on-site groups, programs, drop-ins, non-essential meetings and walk-ins are suspended until further notice. Groups will continue to be delivered online.

What should I expect when I come for my booked appointment?
All clients with booked on-site appointments, will be asked at reception to sanitize your hands, remove the mask you brought from home and put on a mask that we provide, and then sanitize your hands again.

To help with physical distancing, there are less staff and clients in the Centre at any one time.

All staff are wearing masks and are screened daily to ensure a healthy workplace.

Keep six feet apart from others.

Can I bring a support person with me to my booked appointment?
Only clients that need assistance should bring a support person with them to their booked appointment.

The support person will also be required to complete a Covid-19 assessment, wear a mask, sanitize their hands and provide contact information( name/ phone number).

Stonegate CHC & COVID Vaccinations

January 5, 2021

Stonegate CHC is committed to providing COVID 19 vaccines to our community. The Government of Ontario has begun to implement their three-phased vaccine distribution plan.
 
Phase 1. Limited doses of the vaccine available for health care workers in hospitals, long-term care homes and retirement homes, other congregate care settings and remote Indigenous communities
 
Phase 2. Increasing stock of vaccines, available to all health care workers, residents in long-term care homes, retirement homes, home care patients with chronic conditions and additional Indigenous communities
 
Phase 3. Vaccines available widely across Ontario for anyone who wants to be immunized
 
Stonegate CHC will expect to receive vaccinations when Phase 3 of the plan is implemented. At this time we cannot indicate when we will receive the vaccines but will share information as soon as it becomes available.

Good Food Box Holiday Drive

During this time, we sincerely hope that you and your family have stayed healthy.

Here at Stonegate CHC, we have continued to offer all of our essential primary healthcare services, mental health counselling, nutrition counselling and are finding many new ways to deliver our programming that keep us connected to our community over the phone and with an increasing number of programs that we are live-streaming. We are here for our community! We need to maintain physical distance but are committed to social connection. Our whole team is on the phone, reaching out because we know we are Better Together.

Access to healthy food should not contribute to problems posed by COVID-19 and thus, since the start of the pandemic Stonegate CHC has been partnering with FoodShare to coordinate, purchase and deliver over 1200 Good Food Boxes to those who have identified as being in need of emergency food assistance in our community. We have also distributed over 1000 grocery vouchers to clients in need.
For the past decade, thanks to generous donations to our annual Holiday Drive, Stonegate Community Health Centre has been able to provide Good Food Boxes to neighbourhood individuals and families dealing with food insecurity and hunger during the holidays.

This year, more than ever we are relying on donations from businesses, partners and community residents to meet our goal of providing boxes not just during the holidays but throughout the year to those struggling to put healthy food on the table.

For our December holiday food box delivery, clients will be able to have their produce box delivered directly to their home without having to pick it up at our Health Centre, thanks to FoodShare.

A donation of $22 buys a local family a large size Good Food Box brimming with fresh fruits and vegetables.

The Good Food Box is a non-profit fresh fruit and vegetable distribution system operated by FoodShare Toronto. Every Good Food Box is filled with top-quality fruits and vegetables from local farmers and the Ontario Food Terminal.

  • $50 provides Good Food Boxes to 2 families in need
  • $100 provides Good Food Boxes to 5 families in need
  • $250 provides Good Food Boxes to 12 families in need
  • $500 provides Good Food Boxes to 23 families in need

100% of donations go towards providing fresh healthy food for those in need. To make a donation, please click on the following link or connect with Julia Graham, Community Food Program Coordinator, at 416.231.7070 ext. 229. Charitable tax receipts are available for every donation.

Invitation Stonegate Community Association Virtual Annual General Meeting (AGM)

September 9, 2020

Stonegate Community Health Centre would like to extend an invitation to join us for our Annual General Meeting.

The AGM will be held on Saturday, September 26, 2020 at Noon. This year due to the pandemic, we will be holding our AGM via a Zoom call. If you would like to join in our meeting, please contact Health.promotion@stonegatechc.org to register or call 416-231-7070 ext. 307. You will then be provided the log-in details to the meeting.

Stonegate CHC’s Stage 3 Update

July 31, 2020

It was announced that the City of Toronto would be moving to Stage 3 effective Friday July 31st, 2020 at 12:01 a.m.

Additional business and public spaces will be able to gradually reopen with public health and workplace safety restrictions in place, while some high-risk venues and activities will remain closed until they can safely resume operations.

What does Stage 3 mean for us?

Stonegate CHC program and services will continue to be implemented on a modified schedule that includes:
- Modified hours Monday to Friday 8:30 a.m. to 4:00p.m.
- All appointments must be scheduled with a provider, if you need to come to the Health Centre your provider will notify you.
- Programs will be provided online.
- All clients will be screened first on the phone when making an appointment with a provider and then by staff behind the glass barrier at reception.
- Clients will be asked to sanitize their hands and wear a mask. Wearing a mask or face covering has been required in indoor public spaces beginning July 7, 2020 under City of Toronto By-Law 541-2020
- Waiting room chairs have been minimized and spaced to ensure 2 metre distance.
- Waiting room will regularly be cleaned by staff wearing full PPE.
- Staff will be screened for symptoms and have their temperature taken at the beginning of each day.

We are exploring providing outdoor programming and are implementing a risk assessment on all our programs and service reviewing how to mitigate risk and determine which programs can safely return.

Stonegate Community Health Centre will continue to work toward recovery acting responsibly by putting measures in place to keep the community safe

What can you do?
- Maintain 2-metres distance from everyone outside your safe social circle.
- Wash your hands often with soap and water or use hand sanitizer.
- Wear a non-medical mask where maintaining physical distancing is difficult and where masks are mandatory. Protect the supply of medical grade masks for health care workers.
- If you think you might have COVID-19 or have been exposed to it you should get tested. While waiting for test results, stay home, self-isolate and prevent potential spread.
