West Parking Lot Closure – January 2023
Due to roadway reconstruction the West Parking lot and roadway access will be affected for approximately a month starting January 9th. We apologize for any inconvenience this may cause.
Stonegate CHC – Friday, December 23rd
Stonegate CHC was scheduled to be open on Friday, December 23rd from 9:00 am – 12:00 pm. Due to the weather advisory posted for the City of Toronto, Stonegate CHC will be closed for in person programs and services.
All in person appointments will be virtual. We can be contacted by phone on Friday, December 23rd between 9:00 am – 12:00 pm at 416-231-7070.
If you have an emergency when clinic is closed please call us at 416-231-7070 and your call we be transferred to our after-hours line.
The Dental clinic will remain open, if you have an appointment please call 416-338-1120.

Please note Online Booking is for current primary care clients that have OHIP.
• Currently you can only book an appointment with Dr. Shannon Cohane, Teresa Cristina Hermenegildo and Crystal Lawrence.
• If this is an emergency, please go to your local ER.
• If you need to book an appointment for a Flu shot, General Check-up/Physical, Well Baby/Child visit or if it is urgent, please call our office @ 416-231-7070 and a Medical Office Assistant will book your appointment appropriately.
• If you are booking an appointment for a family member, please have them book their own appointment if they are over the age of 16yrs old
• Each person will need their own appointment slot
• If the online booked appointment does not meet the criteria, the appointment will be cancelled. We will make our best efforts to contact you but in the event that we cannot, we will re-direct on arrival as appropriate.
• If you need to change or cancel your appointment, please call our office at least 24hrs prior at 416-231-7070 for assistance
